Inquiry #1: I see no pay items for the 3" DWR electrical conduit depicted on plan sheet 50, see note 8 as well as pier 2 typical Section on sheet 50. In addition, the same is applicable to note 6, 5" Fiber optic conduit, no pay item. So reference sheets 50, 59 & 60, please provide pay items for these electrical items of work. Thank you.

Inquiry #2: Per Plan Sheet "Verification Test Details", "The hydraulic jack, load cell, reaction beam, cross beam, associated connections and testing equipmnent to be supplied by the Engineer. The Load Test will be performed by the Engineer". Please confirm how long a "tail" the micropile bar will need to accomodate the Engineer's load testing setup.

Inquiry #4: Per Plan Sheet "Verification Test Details", it appears that the micropile casing extends above OG and has both flange plates and bearing plates at the top to allow for bearing of the test frame elements.
How much does the casing need to extend above OG?
Are the flange and bearing plates covered under the Engineer's furnished connections? If not, please provide details for the flange plates and bearing plates.

Inquiry #6: The maximum allowable tension load on a Grade 150 ASTM A722 bar is 80% of the maximum ultimate tensile load. Plan Sheet No. 58 issued with AD3 lists FTL of 630k for compression and tension, which exceeds the maximum allowable load for a 1-3/4" diameter HS bar listed in the "PILE ANCHORAGE DETAIL", and the 2-1/2" diameter HS bar listed in the "TENSION VERIFICATION LOAD TEST DETAIL". Please confirm the minimum bar size required for the tension verification pile, the reaction piles for the compression verificaiton pile setup, and the production micropiles.

Inquiry #7: We just received Addendum #4, which states "Complete and submit a Prebid Information Qualification Questionnaire form no later than 4 p.m. on Monday, June 24, 2024".

This work requires specialty subcontractors. In order to get competitive pricing on bid day, can we submit multiple qualification questionnaire forms from different subcontractors? How/when will we be notified that these forms that we submit are approved?
